Family Support Affidavit

Support orders, when filed with the court and served on the clerk’s office, can be accompanied by a family support affidavit, which is a preprinted court form that contains identifying information (address, employment, social security number, date of birth, etc.) about both parents. A sample of such a form is included in §4.85 below. Note, however, that these forms are no longer required, and some judges will not accept them, though some apparently still find them useful.
